Skilled Nursing

Because of our size and focus, we have the unique ability to pair you with a primary clinician who will work closely with you and your physician to help you achieve and maintain optimal health. With extensive backgrounds in senior home care, our experienced Registered Nurses will work with your physician to provide the services you need at home:

  • Coordinated recovery from an acute illness
  • Disease Management
  • Post-surgical recovery
  • Pain and medication assessment
  • Wound and ostomy care
  • Intermittent personal care
    (administered by Home Health Aides)

Therapy

North Hill Home Health Care, Inc. provides a skilled therapist with extensive experience in treating a variety of medical conditions in a home setting. Whether your order for rehabilitation originates from a doctor’s office or after a discharge from a hospital or skilled nursing facility, our therapists will create a personalized plan of care to optimize your recovery.
Our Physical Therapists work with your physician to design a program that can comfortably and conveniently be performed in your home to help you achieve the greatest possible level of physical function.
Should you require adaptations to perform the activities necessary to maintain independence in your home, our Occupational Therapists will use assistive devices, exercise and other treatments to optimize your safety so you may resume or maintain your daily routines.
If your speech or other functions have been impaired, our Speech Therapists will help you regain control and improve the quality of your speech. This will help with understanding language, and with swallowing problems, as well as articulation, voice disorders, fluency, cognitive and communication impairment.

Medical Social Work

Our medical Social Worker is an expert in meeting the needs of seniors in the home care setting. Working collaboratively with your clinical team, the Social Worker’s goal is to help you, and your family adjust to changes in your health and living choices. Home Health Aide Services

North Hill Home Support is the ideal team to collaborate with to assist in maintaining your healthy independent lifestyle. Our team consists of Registered Nurses, Certified Home Health Aides, and Speech, Occupational, and Physical Therapists. We are here 7 days a week. All of our team members are employed by North Hill. It is the goal of Home Support to provide ongoing health care management, education, and oversight to all our clients.

The Registered Dietitian is a health professional who will provide nutrition assessment and education. The goal is to promote health, prevent disease, and shape dietary choices for residents who are both healthy and experiencing illness. The Geriatric Care Manager is a qualified professional with a clinical background that completes an assessment and manages care needs. The services provided are the following:

  • Evaluating and arranging for hired caregivers
  • Coordinating medical appointments and arranging transportation
  • Identifying social services and programs that support each individual
  • Explaining complex or difficult topics
  • Assisting with long-term and short-term care plans
  • Assist with long-term care insurance, access/coordination of benefits
  • Liaison for families
  • Assist with coordinating respite care
  • Provides personalized assistance and support
